Introducing Our Collaborative Change Model
We are excited to launch our new Collaborative Change Model – a bold community-driven roadmap designed to amplify our collective impact and advance our mission to eliminate poverty in Guelph and Wellington County. This model is not just a plan; it is a commitment to collaborative upstream action and centring intersectional lived experience.
A Vision Shaped By Community
Over the past two years, we have engaged with community partners, leaders, key players from diverse sectors and individuals with lived experience to co-create a shared vision for a future where community members have their long term housing, income, food, employment, transportation, and health needs met. The result is a model that reflects our values, shared purpose, priority issue areas, and collaborative upstream approaches to eliminate poverty.





Why This Matters
Our Collaborative Change Model and Activation Framework is more than a strategy – it provides clear objectives and actionable upstream approaches to address the root causes of poverty. Most importantly, it centers intersectional lived experience to ensure all aspects of our work is grounded in the realities of those most affected by poverty. By aligning our collective efforts, together we can build a more just community where everyone can experience equitable outcomes, care, belonging, wellbeing and value in a vibrant community.
